Flu Shot Clinic

Blanco ISD is partnering with Blanco Pharmacy & Wellness to offer flu shots for all BISD staff and students aged nine and older.
To participate, please submit a completed consent form by Tuesday, October 14th.

Where to find the form:

• On the BISD Website (look for the file titled "Screening Checklist for Contraindications to Vaccines for Adults").
• At the front office of any BISD campus.

To secure a flu shot for your child, please follow these steps:

• Complete the Screening Checklist form.
• Attach a copy of your insurance card or Medicaid card (front and back).
• Return the completed paperwork to the front office of your child's campus.
• You can also email the completed forms to angela.kuzins@blancoisd.org.
  
  No Insurance/Medicaid? The cost is $35 per shot, which must be paid in advance.
